Notice of Data Security Incident

Gilbert, Arizona – October 18, 2023 – The Arizona State Urological Institute (“ASUI”) notified certain current and former patients that their personal information may have been accessed as part of a cybersecurity incident. ASUI takes the privacy and security of information in its possession very seriously and sincerely apologizes for any inconvenience this incident may cause. This notice is intended to alert potentially impacted patients of the incident, steps we are taking in response, and resources available to assist and protect individuals.

What Happened On or around May 8, 2023, one of ASUI’s employee’s sent certain documents containing patient information to her personal email account in violation of ASUI policies prohibiting such conduct. Immediately upon learning of this incident, ASUI launched an investigation into the employee’s conduct. The protection of patient information is of the utmost importance to us. For this reason, we retained legal counsel to assist in response efforts, and reported the incident to law enforcement authorities. The employee is no longer employed by ASUI.

As of this writing, ASUI has not received any reports of related identity theft or other misuse of patient information since the date of this incident (May 8, 2023 – present). ASUI is providing written notice to all impacted patients.

What Information Was Involved Although we have found no evidence that specific information has been accessed for misuse, it is possible that your name, date of birth, and/or protected health information, to the extent that such may have been contained in the documents, could have been exposed.

What We Are Doing Data privacy and security are our highest priorities. Upon detecting this incident, we have taken steps to prevent a similar incident in the future by implementing additional safeguards and employee training. We have also reviewed and taken steps to enhance our policies and procedures relating to patient information, as well as our information life cycle management.
